It is advisable to get your TB certificate ready so you can apply for visa quickly. It is definitely mandatory. You need to TB free before travelling to UK. Your and dependents TB clearance can only be done at
certain centers in Pakistan mentioned on the UK visa website here. For Karachi its located on Shahrah-e-Faisal behind KFC. The department is known as IOM (International Organization for Migration) you need to call / email them your details including name, address, passport details and address and phone number of your  employer. Once your appointment is confirmed you will asked to pay a bank challan of staggering 8k-9k. Bank challan are available only at certain Standard Chartered Bank (shaheed millat road, Shahrah-e-Faisal, university road opp Bait ul Mukaram). With that challan, original passport, nic and 3 photographs reach the IOM office where your details will be taken and explained and asked about history of TB. A Chest Xray will be done after this and you will be asked to return in the afternoon the same day to collect your TB certificate provided Xray are normal. One important point is if your wife is expecting you need to let them know you will be given two options either delay it till 2nd 3rd trimester or till the end of pregnancy or get it done immediately after signing a consent form that IOM will not be responsible for any harmful effects of radiation to the fetus. Remember you cannot travel without TB certificate. They will provide a lead shield provided you sign the consent form. No letters from doctors previous Xray are acceptable as alternative.
